[dynamic_form] form name or display name not rendered in zeppelin ui

When using dynamic forms in a notebook , the form name or even display name is not rendered in the UI.

Example:

(From stock example notebook titled "Using Pig for querying data")

The form name maxAge should be displayed above the input box.
