[MSVC] use __declspec(noreturn) where __attribute__((noreturn)) is used

As noted in this comment on STDCXX-890, we can and should use Visual C++ __declspec(noreturn) to declare functions that do not return to the caller to avoid warnings and to help the optimizer generate better code.
