CloudSolrStream silently returns empty stream when all nodes are down

When reading from a `CloudSolrStream`, if all Solr cores are down, we receive an empty stream (EOF from the first `read()` operation).

Instead, an exception should be thrown to represent that the Solr cluster is unreachable, even if the ZooKeeper cluster itself is reachable.
