Change documentation links to "*.md"

Right now, links either use the form

[label](/documentation/latest/foo/)

or

[label](foo.md)

. We should probably switch to using the latter form consistently – it previews better on Github, and it will make it easier to have multiple versions of the docs on the website at once in the future.
