Improve the UI to add files to a list

In a number of transform dialogs, like for example the Text File Input dialog, you have the ability to add filenames to a list.  This user interface is not intuitive and could be improved a lot to avoid confusion.

Most of the errors occur when a person forgets to hit the "add" button after "browse".

 
Can we put the browse button somewhere else?
Can we add the browse button result(s) to the list or edit it automatically when a line is selected?
IMO we can get rid of the top 3 lines and move the browse button to the right.
