Enrich trace span information

As I'm working on adding usable Trace data to Phoenix, I find that HBase trace spans are missing crucial information.

This an umbrella ticket to track those issues, and add crucial information that is needed for interpreting trace data.

The goal is to add the most useful data while keeping the Span data as compact as possible.
