Write response directly instead of creating a fake call when setup connection

We do not execute the 'call' with CallRunner, so it is not necessary to create it. We can remove several fields in RpcServer.Connection, and we can also remove setSaslTokenResponse and setConnectionHeaderResponse in RpcServer.Call.
